On the evening of 29th of June 1968, Constable Sullivan was on Police motorcycle patrol duty in the Victoria Park area.

Just before midnight, near the corner of Albany Highway and Cargill Street, Victoria Park, his motorbike collided with a car traveling in the opposite direction.

Constable Sullivan suffered serious injuries and was rushed to Royal Perth Hospital but was pronounced dead on arrival at 12.20am.
