On the 23rd of January 1971, Constable O'Sullivan was killed, whilst he and his colleagues were conducting amphometer speed checks near Lake Monger, Wembley. 

Whilst interviewing the driver of a vehicle, Constable O'Sullivan was struck by another vehicle traveling in the opposite direction.   

Constable O'Sullivan died instantly at the scene.
