On the 1st of December 1937, Inspector Lewis, Officer in Charge of the Traffic Branch, died from injuries sustained in a traffic crash.

The crash occurred when his vehicle was hit by another car, causing it to overturn at the intersection of fifth Avenue and Robinson Streets, Maylands.

Inspector Lewis was rushed to Perth Hospital suffering from severe head injuries but died shortly after admission.
