On 7th of March 1961, Constable Flatt, was stationed at Northam Police Station, died due to injuries sustained from a traffic crash that occurred near Dalwallinu.

Constable Flatt and another officer were conveying a prisoner by ambulance to the Dalwallinu Hospital, when their vehicle rolled, although injured, all survived the initial crash.

Another ambulance arrived, but could only take two patients at a time, Constable Flatt volunteered to remain and wait for the ambulance officers to return, when they returned, they found Constable Flatt had died from internal injuries.
