On 13th of July 1995, Constable Duckham was driving an unmarked Police vehicle with emergency lights and sirens activated, responding to a call for assistance from colleagues dealing with a violent domestic disturbance in Clarkson. 

At the intersection of Marmion Avenue and Burns Beach Road, Joondalup, Constable Duckham’s vehicle collided with a truck that had turned onto Marmion Avenue.

Constable Duckham died instantly at the scene.
